Pathology Referral Form

We strive to provide the best possible service but:
• There is always a chance of error in pathology diagnosis, however, this is a rare event and is reported up to 3 to 4 percent of cases. Some of these are minor and inconsequential. The error rate of the pathologist examining your tissue has been less than half of 1 percent overall and less than 1 in 1,000 in gastrointestinal cases.
• There is always a chance of disagreement among pathologists (about 2 percent of cases), even among experts. However, major harm due to these disagreements in diagnosis is rare.
• It is a routine practice to submit difficult pathology cases to a second opinion by another pathologist, or to seek opinion of an expert pathologist in the field. If you are utterly concerned about the accuracy of your tissue diagnosis and you would like an expert see your slides, your pathologist will make the arrangements, provided that you’re willing to pay for their service in advance.
• In case of a major error in your pathology diagnosis, your doctor will be notified at once, a revised report will be issued, and you will not be billed for our service.
• It is always preferred that you discuss pathology diagnosis issues with your doctor because they know all about your illness. However, feel free to talk to your pathologist if you have any other related questions.
